CODE VEIN II features a rich narrative, where players and characters are linked by blood, fate, and fate, set in a future where the last remnants of humanity and the Revenants struggle against a world on the brink of collapse. Following the sudden appearance of a threat known as the Luna Rapacis, the Revenants have succumbed to madness, transforming into terrifying creatures called Horrors. Players, as the Revenant Hunters, will be guided by a mysterious girl named Lou who possesses the power to travel to the past. Both will intervene at crucial moments in history to alter the destinies of the Revenants and to reveal hidden truths and information, thus rewriting the fate of the world and saving the future.

Players will embark on a journey through a world that spans two eras, where decisions matter and each battle tests skill and determination. CODE VEIN II has distinctive Action-RPG gameplay, where players use blood obtained from enemies to unlock powerful abilities, build strategies for combat, and use unique weapons and equipment called Jails to overcome challenging enemies and epic bosses.
